This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Please only complete this form if your student is new to the Slinger School District.  Complete the required fields to request a Family Access account. 
If you have already started the application process please refer back to the email you received for the URL and your login information. 
For families with students in the district already, please access your Family Access account and use the NEW Student Enrollment tab.
